Our providers are specially trained to work with people who struggle with substance use. We offer a range of therapeutic interventions and medical options to help you navigate your recovery. We work closely with you to ensure you get the safe, effective treatment you need.
Our Providers
Joyce Troxler, M.D.
Dr. Joyce Troxler, M.D. is dually board certified in Family Medicine and Addiction Medicine. Her personal history is that of a rural upbringing in Central Appalachia. Dr. Troxler gained the majority of her clinical experience in a rural community in Southwestern New Mexico. Her practice in rural New Mexico was largely focused on providing treatment for Substance Use Disorder.
Caleb Osborne, M.D.
Dr. Caleb Osborne felt led to medicine to help people enact generational and community change. He and his family fell in love with the area after they moved here for his family medicine residency. Dr. Osborne believes that you have infinite value as a human being, and he will treat you with love and respect no matter what you're going through.
Johana Zambrano, D.O.
Treatments
- Medication Management
- If medication is indicated, our board certified addictionologists will talk with you about the options and help you formulate a treatment plan that fits your needs and serves your goals. They will work closely with you to ensure proper use of medication and closely monitor for any medication issues.
We also offer medication-assisted treatment for multiple substance use disorders: including nicotine, stimulants, opioids, alcohol and more.
- Primary Care
- We universally screen for hepatitis c. If confirmed that you have an active infection, we can provide the treatment for you.
- Behavioral Health (Individual, Couples, Family & Group Therapy)
- Individual, couple, and group therapy is available, depending on your specific needs. In all cases, our specialists work with you to help you manage and control your addiction and substance abuse disorder. We use motivation approaches that include a range of cognitive behavioral therapies.

Comprehensive Evaluation
We gather information on your medical history, psychological history, goals, and values,
then work closely with you to create a treatment plan that has clear targets for change.
We help you understand how your substance use developed and how we can work together
to effect change. We will also assess for general health and well-being, performing
examinations as indicated and collecting lab work.

Specialists in Substance Use Disorders
Our clinic is staffed with an integrated team of providers and learners. Addiction
medicine is a newer specialty recognizing the complicated course of substance use
disorders and behavioral addictions. In addition to our board certified addictionologists,
we have physicians training in our addiction medicine fellowship to become addictionologists.
The specialty of addiction medicine requires that the physician is board certified
in a primary specialty. Our addictionologists are also board certified in family medicine
or psychiatry.
In addition to our physicians, we have a variety of specialists in the behavioral
health field. Their credentials range from social work (LCSW, LBSW, MSW) to professional
counselors and certified peer recovery specialists. These talented individuals can
provide counseling and case management as well as peer support.

We offer a full range of therapy options, including individual, group and family therapy.
Referrals
If there are services or a higher level of care that is needed, we can facilitate
your referral so you can start your individualized treatment as quickly as possible.
Support and Long Term Follow-Up
Our clinic does not run on a predetermined timeline. You will have our help and support
for the time you need it. Our goal is to assist you in achieving your goals. When
needed, we help you find additional resources to support your recovery, decrease risk
of return to use, but should there be return to use, you do not lose our support.
