Director of Population Health & Quality Improvement
Job Description
SUMMARY:
The Director of Population Health is responsible for all aspects of the Population
Health department, including development of strategic vision, working with value-based
partners, and managing the daily aspects of the department. Reporting to the Chief
Medical Officer, the Director drives and reports on effective execution of all value-based
agreements and services throughout the organization. The Director oversees all activities
of the department and manages team members, which include data analysts. case managers,
and population health analysts.
ESSENTIAL DUTIES AND RESPONSIBILITIES:

- Develops and implements payment and reimbursement strategies in alignment with the organization's vision, goals, and strategy.
- Team management, which includes process and workflow development to ensure success in value-based programs; may include development of specific reports to assist with operational implementation of programs in the clinic setting; development of a new case management workflow to capture additional data necessary for payer incentives, or creation of new processes within the clinics or Population Health department to enhance reimbursement from a value-based program.
- Develops reports to analyze and evaluate the effectiveness of all value-based programs, and shares with various department and administrative leadership teams on a routine basis.
- Develops and performs ongoing quality education for physicians, residents, and staff throughout the year.
- Responsible for meeting reporting requirements for all value-based, State, and Federal programs and assures all programs remain compliant with payer, State and Federal regulations.
- Investigates, develops, and implements new value-based programs as appropriate.
SUPERVISORY RESPONSIBILITES:
Directly manages Data Analysts, Case Managers, Population Health Analysts. Carries out managerial responsibilities in accordance with the organization’s policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.
EDUCATION AND EXPERIENCE REQUIREMENTS:
- Bachelor’s Degree with 3 to 5 years of mid- to senior-level management experience in healthcare, or an equivalent combination of education and experience, required.
- Clinical healthcare experience preferred, such as Registered Nurse.
- Knowledge of value-based programs, contracts, and population health required.
- Excellent verbal, written and interpersonal communication skills.
- Excellent analytical and organizational skills with strong attention to detail.
- Demonstrates strong leadership skills.
- Ability to function effectively within an ever-changing environment to meet deadlines and reprioritize as necessary.
- Proven ability to solve problems and make sound decisions.
- Project Management skills preferred.
- Ability to work independently and within a team environment.
- Proficiency in MS Office (Excel, Word, and PowerPoint).
- Experience with SQL, Access, or other data analytic tools preferred.
